Hacimden değere : değer temelli sağlık hizmeti

Abstract

Günümüzde birçok ülke, sağlık hizmetleri maliyetlerinin artması, yaşlanan nüfus ve kaynakların kıtlığı gibi nedenlerden dolayı yüksek kalite ve maliyet verimliliği sağlamak için sağlık hizmetlerini nasıl yönlendireceğini araştırmaktadır. Gerek bu araştırmalar gerekse de artan maliyet ve kalite baskısı, sağlık hizmetleri sunumunu uzun süredir devam eden hacme dayalı sağlık modellerinden "değer temelli sağlık hizmeti" modellerine yönlendirmektedir. Çünkü değer temelli sağlık hizmetleri, hastaların daha uygun maliyetle, koordineli, uygun ve etkili bir bakımı alabileceği, bireylerin ve toplumların sağlığını iyileştirebileceği bir bakım deneyimi sunmaktadır. Bu anlamda birinci amaç, sağlık hizmetlerine sağlayabileceği potansiyel kazanımların teorik olarak ortaya konulmasıdır. Dolayısıyla literatürde yeni bir bakış açısı ve yaklaşım olan değer temelli sağlık hizmetinin tanımlanarak açıklanması, bu konuda kuramsal bir çerçeve oluşturulması hedeflenmiştir. Ayrıca bu tez ile daha önce üst düzey sağlık yöneticiliği yapmış veya halen yapmaya devam eden sağlık yöneticilerinin, değer temelli sağlık hizmetlerine ilişkin görüşlerinin incelenmesi de amaçlanmıştır. Bu amaç doğrultusunda katılımcıların özgeçmişleri ve sahip oldukları unvanlar itibari ile kimliklerinin ortaya çıkmaması için çalışmanın yapıldığı ilin ismi gizli tutulmuştur. Nitel araştırma yönteminin kullanıldığı çalışmada, belirlenen ilin merkezinde amaçlı örnekleme yöntemlerinden kolay ulaşılabilir örnekleme ve ölçüt örnekleme yöntemi ile ulaşılan 7 üst düzey yönetici ile görüşmeler gerçekleştirilmiştir. Çalışmada ilgili literatür taranarak araştırmacı tarafından oluşturulan görüşme formu kullanılmıştır. Araştırma sonucunda, hasta sağlığına zarar vermeden maliyetlerin azaltılarak daha kaliteli ve verimli sağlık hizmeti sunulmasının gerekli olduğu, bunun da değer temelli sağlık hizmetli ile sağlanabileceği sonucuna ulaşılmıştır. Hasta, sağlık personeli ve kurum açısından değerin arttırılması ve değer temelli sağlık hizmetlerinin bileşenleri olan liderlik ve teknoloji ile bakımın desteklenmesinin önemi vurgulanmıştır. Yeterli fiziksel koşullara sahip, katılımcı, ölçülebilir ve denetlenebilir bir sistemin yapılandırılması ile değer temelli sağlık hizmetlerinin sunumunun mümkün olabileceği ulaşılan diğer bir sonuçtur. Ayrıca eğitim ile nitelikli personele sahip olunması, doğru yatırımların yapılması ve gereksiz hizmetlerin azaltılmasına ilişkin yapılacak düzenlemeler ile değer temelli sağlık hizmeti sunumuna dönüşüm sağlanabileceği ve sağlık sisteminin bu geçiş için hazır olduğu, katılımcı çoğunluğu ile elde edilen diğer bir sonuçtur.

Nowadays, many countries are now investigating how health care costs can be directed to ensure high quality and cost efficiency due to increased health care costs, aging populations and scarcity of resources. Both these researches and the increasing cost and quality pressure lead the delivery of health services to, value-based healthcare models of long-standing volume-based health models. Because value-based healthcare provides a care experience where patients can receive more cost-effective, coordinated, appropriate and effective care, and improve the health of individuals and communities. In this sense, the first aim is to put forth the theoretical gains that can be provided to health services. Therefore, it is aimed to define and explain the value-based health service which has a new perspective in the literature, and to form a theoretical framework on this subject. In addition, this thesis aims to examine the opinions of the health managers who have previously made a high level health management or who are still doing this, on the value-based health care. In accordance with this purpose the name of the province where the study was conducted was kept confidential in order to ensure that the identities and titles of the participants were not understood. In the study using qualitative research method, in the center of the identified province, interviews were conducted with 7 senior managers who were reached through convenience sampling and criterion sampling methods. In this study, the related literature was searched and the interview form created by the researcher was used. As a result of the study, it was concluded that the costs should be reduced by providing better quality and efficient health services without harming the health of the patient and this could be achieved by value-based healthcare. The importance of increasing the value in terms of patient, health personnel and institution and supporting the maintenance with leadership and technology which are the components of value-based healthcare were emphasized. Another result is the provision of value-based healthcare through the establishment of a participatory and measurable and auditable system and ensuring adequate physical conditions. In addition, another result obtained by the majority of the participants is that the health system is ready for this transition and that it can be transformed into value-based healthcare through arrangements to be made with qualified personnel, and reducing unnecessary services.
