Abstract

AMAÇ: Perioperatif kanama yönetiminde en önemli basamaklardan birisi risk altındaki hastanın önceden belirlenmesidir. Major kanamayla karşılaşılabilen vertebra cerrahilerinde literatürde bugüne kadar intraoperatif kanamada prediktivite açısından trombosit belirteçlerini inceleyen bir araştırma bulunmamaktadır. Bu çalışmada; vertebra cerrahisine girecek hastalarda preoperatif ortalama trombosit hacmi (MPV), trombosit dağılım genişliği (PDW), plateletkrit (PCT), ortalama trombosit hacmi lenfosit sayısı oranı (MPVLR) ve trombosit ve lenfosit sayısı oranı (PLR) değerlerinin intraoperatif kanama miktarı tahminindeki etkinliklerini araştırmayı amaçladık. METOD: Etik kurul onayı sonrası hastanemizde beyin cerrahisi tarafından vertebra cerrahisi planlanan hastalar belirlendi. Çalışmaya dahil edilen hastaların cinsiyet, yaş, ASA skoru ve vücut kitle indeksi (VKİ) gibi demografik verileri; operasyon öncesinde alınan hemogramdan MPV, PDW, PCT, MPVLR ve PLR değerleri kaydedildi. Toplam cerrahi süresi, vertebra transpediküler vida ve laminektomi seviye sayıları kaydedildi. Ameliyat süresince aspiratörde biriken kan ve spançlardaki kan miktarı üzerinden gerekli hesaplamalar yapılarak intraoperatif kanama miktarı elde edildi. BULGULAR: Çalışmaya dahil edilen 63 hastanın %60,3'ü (n=38) kadın, %39,7'si (n=25) erkek idi. Hastaların yaş ortalamaları 54,7 ± 11 yıl, ortalama VKİ 29,6 ± 4,6 idi. Cinsiyet, yaş, trombosit belirteçleri (MPV, PDW, PCT, MPVLR ve PLR değerleri) ile kanama miktarı arasında korelasyon saptanmadı. VKİ, cerrahi süresi, transpediküler vidalama segment sayısı ile kanama miktarları arasında ise anlamlı derecede yüksek korelasyon saptandı. SONUÇ: Sonuç olarak; intraoperatif kanama miktarı ile hastanın cinsiyeti, yaşı, MPV, PDW, PCT, MPVLR ve PLR değerleri arasında korelasyon saptayamazken; hastanın VKİ'si arttıkça, cerrahi süre uzadıkça, uygulanan transpediküler vida segment sayısı arttıkça kanama miktarının arttığını saptadık. EVALUATION OF THE EFFECT OF PLATELET MARKERS ON THE AMOUNT OF INTRAOPERATIVE BLEEDING IN VERTEBRA SURGERY AIM: One of the most important steps in perioperative bleeding management is the predetermination of the patient at risk. In vertebral surgeries where major bleeding can be encountered, there is no study in the literature examining platelet markers in terms of predictivity in intraoperative bleeding to date. In this study; We aimed to investigate the efficacy of preoperative mean platelet volume (MPV), platelet distribution width (PDW), plateletcrit (PCT), mean platelet volume lymphocyte count ratio (MPVLR) and platelet and lymphocyte count ratio (PLR) values in predicting the amount of intraoperative bleeding in patients who will undergo vertebral surgery. METHOD: After the approval of the ethics committee, the patients who were planned for vertebral surgery by the neurosurgeon in our hospital were determined. Demographic data such as gender, age, ASA score and body mass index (BMI) of the patients included in the study; MPV, PDW, PCT, MPVLR and PLR values were recorded from the preoperative hemogram. Total surgery time, vertebral transpedicular screw and laminectomy level numbers were recorded. The amount of intraoperative bleeding was obtained by making necessary calculations on the amount of blood accumulated in the aspirator during the operation and the amount of blood in sponge. RESULTS: Of the 63 patients included in the study, 60.3% (n=38) were female and 39.7% (n=25) were male. The mean age of the patients was 54.7 ± 11 years, and the mean BMI was 29.6 ± 4.6. No correlation was found between gender, age, platelet markers (MPV, PDW, PCT, MPVLR and PLR values) and the amount of bleeding. A significantly high correlation was found between BMI, duration of surgery, number of transpedicular screwing segments and amount of bleeding. CONCLUSION: As a result; While there was no correlation between the amount of intraoperative bleeding and the patient's gender, age, MPV, PDW, PCT, MPVLR and PLR values; We found that the amount of bleeding increased as the patient's BMI increased, the duration of surgery increased, and the number of transpedicular screw segments applied increased.
