GİRİŞ VE AMAÇ: Sezaryen, gebelik ya da doğumda, herhangi bir komplikasyon gelişmesi durumunda anne ve bebeğin hayatını kurtaran ve dünyada en sık yapılan cerrahi girişimlerden biridir. Dünya Sağlık Örgütü'nün (DSÖ), 1985 yılında ideal sezaryen hızının, tüm doğumlar içinde %10-15 olması gerektiğini belirttiği tarihten bu yana sezaryen hızı hem dünyada hem de ülkemizde kontrol edilemez bir hızla yükselmektedir. Yüksek sezaryen doğum hızı, yalnızca tıbbi endikasyonu olmayan müdahalelerden kaynaklanan kısa vadeli maternal ve perinatal komplikasyonlar nedeniyle değil, aynı zamanda uzun vadeli komplikasyonlar ve maliyet artışı nedeniyle de önemli bir halk sağlığı sorunu oluşturmaktadır. Bu çalışmada kadın doğum hekimlerinin görüşlerinden yola çıkarak sezaryen doğum hızının nedenleri ve olası çözüm önerilerinin saptanması amaçlanmıştır. GEREÇ VE YÖNTEM: Yöntem olarak nitel (kantitatif) araştırma yöntemi ve nitel araştırma deseni olarak fenomenolojik yaklaşımın kullanıldığı bu çalışma, Marmara bölgesinde yer alan, Türkiye'nin 30 büyük şehrinden biri olan Sakarya ilinde 01.Şubat.2020 ile 01.Eylül.2020 tarihleri arasında yürütülmüştür. Çalışmada görüşülecek kişilerin seçilmesinde olasılıksız örnekleme yöntemlerinden amaçlı örnekleme yöntemi kullanılmıştır. Çalışmanın katılımcıları, Kadın Hastalıkları ve Doğum branşında uzmanlık eğitimini almış (en az dört yıl) ve en az üç yıl uzman hekim olarak çalışmış ve halen çalışmakta olan kadın hastalıkları ve doğum uzmanlarından seçilmiştir. Çalışmada, çalışma amacına uygun olarak derinlemesine görüşme yoluyla veri toplanmıştır. Görüşme esnasında katılımcılardan ses kaydı için izin alınmış olup, izin vermeyen altı katılımcıyla yapılan görüşmelerde not tutma yöntemi kullanılmıştır. Katılımcılarla görüşmelerde iki bölümden oluşan bir anket formu kullanılmıştır. Birinci bölüm altı sorudan oluşan sosyodemografik verileri içermekte olup, ikinci bölüm literatür taramasının ardından hazırlanan yarı yapılandırılmış sorulardan oluşan görüşme anketidir. Katılımcılarla yapılan xv görüşmeler esnasında alınan ses kayıtları, araştırmacı tarafından word programı kullanılarak bire bir yazılı hale getirilmiştir. Yazılı verilerin analizinde tematik içerik analizi yöntemi kullanılarak alt tema ve temalara ulaşılmıştır. BULGULAR: Çalışmada 17 kadın hastalıkları ve doğum uzmanı ile derinlemesine görüşmeler yapıldı. 17 katılımcının 12'si erkek (%70,6), 5'i (%29,4) kadındı. Yaş dağılımında minumum ve maksimum değerleri 42-68 olan katılımcıların yaş ortalaması ve standart sapması 59,64±6,7'ydi. Katılımcıların 8'i (%47,1) gelir durumunu iyi, 9'u (%52,9) orta olarak beyan etmişlerdir. Bu görüşmeler sonucu elde edilen verinin tematik içerik analizi sonucunda, 60 kategori, bu kategorilerden 16 alt tema ve bu alt temalardan 4 ana tema oluşturuldu. Temalar: Yüksek sezaryen hızının nedenleri, sezaryen doğum hızında artışa çözümler, sezaryen doğumun olası sonuçları, Türkiye'de Kadın Hastalıkları ve Doğum uzmanı olmaktı. SONUÇ: Türkiye'de ve dünyada her geçen yıl sezaryen doğum hızının artması nedeniyle, bu çalışmada kadın doğum hekimlerinin görüşlerinden yola çıkarak sezaryen doğum hızının nedenleri ve çözüm önerileri saptanmıştır. Nedenler arasında sağlık sistemi ve sağlık politikaları, hukuk sistemi, hekimler ve hastalarla ilişkili faktörler ön plana çıkmaktadır. Çözüm önerileri olarak da sağlık ve yargı sisteminde düzenlemeler yapılması, ebelik sisteminde değişiklikler yapılması ve hekimlere destek sağlanması ön plana çıkmaktadır. Sezaryen hızına etki eden faktörler çok yönlü ve birbirleriyle bağlantılı olduğu için, önlem alınırken bu çok yönlülük ve bağlantı dikkate alınmalıdır. Özellikle sezaryen hızındaki artışta büyük payı olan tıbbi endikasyonları olmayan sezaryan doğumlara yönelmemiz gerekmektedir. INTRODUCTION AND AIM: Introduction: C-section is one of the most common surgical procedures and is done to save the lives of the mother and baby in cases of complicated labour and pregnancy. Since World Health Organization (WHO) in 1985 declared that the C-section should be between 10-15 percent of all the deliveries, the number of operations have raised uncontrollably. High C-section delivery percentage doesn't only cause short-term perinatal and maternal complications due to unindicated operations but also long-term complications, public health problems and raise in the health expenses. This study is focusing on what the obstetricians think about the causes of raise in C-section numbers and their ideas on the possible solutions. MATERIALS AND METHODS: This study uses the quantitative research method and quantitative research pattern phenomenologically and is done in Sakarya which is one of the 30 big cities in Turkey and which is situated in the Marmara Region between 01.02.2020 and 01.09.2020. The people to be interviewed in this study has been chosen by the purposeful sampling, one of the improbable sampling methods . All the obstetricians interviewed in the study have completed a obstetrix and gynecology program (at least 4 year) and worked a minimum of 3 years as a doctor afterwards. The study collects data via deep interviewing method. We have taken the consent from the attendants to record the interviews and for those who doesn't consent to recording have we taken notes. In the meetings with the attendants we have used a questionairre that was made of two parts. The first part consisted of six questions related with the sociodemographical data while the second part consisted of half-constructed questions depending on the literature. The recorded interviews have been written down by the xvii researcher by the help of Microsoft Word. For the analysis of the written data have we used the thematic content analysis and reached subthemes and themes. RESULTS: 17 obstetricians have been deeply interviewed in this study. 12 male (70.6%) and 5 female (29.4%) were interviewed. The age distribution of the attendants were between 42-68 with standard deviation 59.64±6.7. 8(47.1%) of the attendants declared their monthly income as good and 9(52.9%) as normal. After the thematical analysis of the data collected by the interviews with these doctors, we created 60 categories, 16 subthemes and 4 main themes. Themes were: Possible reasons of high C-section rates, doctors' the suggestions for the solution of high C-section rates, possible outcomes of the C-sections, and being a specialist in the obstetrics and gynaecology field in Turkey. CONCLUSION: Because the rate of C-sections raise continously worldwide and in Turkey, we tried to find out what obstetricians think on the reasons of this and their suggestions to solve this situation. According to attendants, the reasons for high rate of C-sections are problems in the health system and politics, the justice system, and the doctor-patient relations play the biggest roles. Suggestions of the doctors were making corrections in the health and justice systems, solving problems in the midwifery system and supporting the doctors. The reasons contributing to the hig rate of C-sections are multifactorial and related to each other so the measures to be taken should also be comprehensive. We should specially focus on the operations which raise the rate of C-sections but medical indications are weak.
