Amaç: Yaşa bağlı makula dejenerasyonu (YBMD) ile ilişkili retina pigment epitel dekolman (PED) olgularında intravitreal ranibizumab ve aflibercept tedavisinin anatomik ve görsel sonuçlar üzerine etkisini değerlendirmek. Gereç ve Yöntem: Kliniğimiz Retina Birimince Ocak 2012 ve Ocak 2017 tarihleri arasında takip edilen YBMD ile ilişkili PED'i olan, ranibizumab veya afliberceptin monoterapi veya ardışık olarak uygulandığı 87 olgunun 98 gözüne ait veriler retrospektif olarak incelendi. Dosya verilerinden ve görüntüleme sistemi kayıtlarından, göz içi basınç (GİB), optik koherens tomografi (OKT) ile ölçülmüş santral retina kalınlığı (SRK), lezyon tipi, lezyon genişliği, PED'in alt tipi, enjeksiyon sayısı, takip süresi ve vizit sayısı değerlendirildi. Enjeksiyon öncesi ve sonrası Snellen eşeli ile ölçülmüş en iyi düzeltilmiş görme keskinlikleri (EİDGK) değerlendirildi. Enjeksiyon sonrası 6.ay ve enjeksiyon sonrası 1.yıl PED taban genişliği ve PED yüksekliği başlangıç değerlerle karşılaştırıldı. Bulgular: Çalışmaya 87 hastanın 98 gözü dahil edildi. Hastaların 41'ü (%47.2) kadın, 46'sı (%52.8) erkek olup yaş ortalaması 69,05±8.54 (50-88) yıl idi. Hastaların ortalama takip süresi 20,6±16,9 ay, ortalama vizit sayısı ise 22,7±16,8 bulundu. Çalışmaya alınan gözlerin %51'inde (50) sağ göz, %49'ında (48) sol göz etkilenmişti. Gözlerin 50'si (%51) psödofak, 48'i (%49) fakikti. Tüm PED'lerin 34'ü (%34.6) fibrovasküler, 32'si (32.6%) seröz, 19'u (19.3%) hemorajik 13'ü (13.2%) drusenoid PED idi. Fundus Flöresein Anjiografi'de (FFA) lezyonların 14'inin (%13.5) ekstrafoveal, 27'inin (%27.5) jukstafoveal, 57'inin (%58.1) subfoveal yerleşimli olduğu saptandı. PED'lerin 16'sı (16.3%) lezyonun 50%'sinden azını, 82'si (83.6%) lezyonun 50%'sinden fazlasını oluşturmaktaydı. Tüm PED'lerin 34'ü (%34.6) fibrovasküler, 32'si (%32.6) seröz, 19'u (%19.3) hemorajik 13'ü (%13.2) drusenoid PED idi. Gözlerin 39'una ranibizumab, 47'sine aflibercept, 12'sine ardışık tedavi uygulandı. Hastaların anti-VEGF enjeksiyonu öncesi görme keskinliği ortalaması 0.887±0.655 LogMAR olup, enjeksiyonu takiben 1.yılda 0.847±0.614 LogMAR idi. Enjeksiyon sonrası 1.yılda tedavi öncesine göre 32 (%32) gözde görme keskinliğinde artış saptanmış, 37 (%37) gözde görme keskinliğinde azalma saptanmış olup, 29 (%29) gözde ise görme keskinliği aynı kalmıştır. Başlangıçta PED yüksekliği ortalama 278.26±171.49µ, PED genişliği ortalama 2547.231±1201.881µ iken, enjeksiyon sonrası 6.ayda PED genişliği 2283.937±1198.778µ, PED yüksekliği ise 272±224,65µ olarak ölçüldü. Enjeksiyon sonrası 1.yılda ise PED genişliğinin 2515.08±1260.10µ, PED yüksekliğinin ise 246.2±176.26µ olduğu görüldü. PED'in ilk rezolusyon zamanı ortalama 15 ay olarak bulundu. Olguların %15-inde PED'in tamamen kaybolduğu, %16-sında ise PED'in halen devam ettiği görüldü. 98 gözün 39'una (% 39.7) ranibizumab, 47'sine (%47.9) aflibercept monoterapisi uygulanmıştı. Her iki grupta da skar ve atrofi gelişti. Lezyonun %50'den büyük olduğu PED'lerde komplikasyon oranı da daha yüksek bulundu. Lezyonunda %50'den büyük PED olan 82 gözün 15'inde (%18.2) RPE rüptürü gelişti. Sonuç: İlk iki senede görme keskinliğinde artış görülmesine rağmen sonraki kontrollerde görme keskinliğinde azalma görüldü. PED taban genişliği 6.ayda başlangıç değere göre anlamlı düşüş gösterirken, 1.yıl kontrollerinde azalma olmadığı görüldü. PED yüksekliğinde 6.ay ve 1.yıl yapılan ölçümlerde anlamlı azalmanın olduğu izlendi. Lezyonun %50'den büyük olduğu PED'lerde daha yüksek oranda RPE rüptürü gelişti. Görsel prognozu belirleyen en önemli faktörler skar ve atrofi gelişmesi olarak gözlenmiştir.
Purpose: To evaluate the effect of intravitreal ranibizumab and aflibercept therapy on anatomical and visual results in pigment epithelial detachment (PED) secondary to age-related macular degeneration (AMD). Materials and Methods: The 98 eyes of 87 patients who had PED secondary to AMD, treated with intravitreal ranibizumab or aflibercept as monotherapy or consecutive therapy were evaluated retrospectively. Best corrected visual acuity (BCVA) evaluated on the Snellen chart, intraocular pressure (IOP), central macular thickness (CMT), lesion type and width, PED height and width, subtype of PED measured with the optical coherence tomography (OCT), number of injections, follow-up duration and number of visits were assessed using patient medical and imaging system records. Baseline and follow-up findings were compared. Results: Out of 87 patients, 41 were female (47.2%), 46 were male (52.8%). The mean age at diagnosis was 69,05 ± 8.54 (50-88) years old. While the mean baseline BCVA was 0.887 ± 0.655 LogMAR, at the end of the follow-up period, it was 0.847±0.614 LogMAR. Among the all PEDs, 34 (34.6%) cases were fibrovascular, 32 (32.6%) were serous, 19 (19.3%) were hemorrhagic and 13 (13.2%) cases were drusenoid. The mean baseline PED height was 278.26 ± 171.49 (113-920) microns; at the end of the followup period, the mean PED width was 2547.231±1201.881 (184-5902). Out of 98 eyes, 39 (39.7%) were treated with ranibizumab, 47 (47.9%) eyes were treated with aflibercept and 12 (12.2%) eyes were treated with anti-VEGF agents. x At the end of the follow-up period, scar and atrophy formation were seen in both groups. Complication rate was higher in PEDs with the lesion greater than 50%. RPE ruptures were seen in 18 of 82 eyes with the PEDs having lesion greater than 50%. Conclusion: While BCVA increased in the first two years, there was a decrease in BCVA in the subsequent follow-up visits. There was a meaningful decrease in PED width comparing baseline versus 6 months, but no decrease comparing baseline vs 12- months follow-up assesment. PED height decreased significantly at 12-months followup from the baseline evaluation. There was a higher percentage of RPE ruptures in PEDs with the lesions above 50%. The scar and atrophy development were determined as the notable factors in visual prognosis
