ÖZET: Anestezi derinliği, Bulanık mantık kontrol, EEG monitörizasyonu. Bu çalışmada, operasyon boyunca anestezi derinliğini kontrol etmek maksadıyla hastadan alman kan basıncı ve EEG kayıtlarını sistem değişkenleri olarak kullanan bir bulanık mantık kontrol sisteminin geliştirilmesi amaçlanmıştır. Operasyon boyunca her beş dakikada bir kan basıncı ve anestezik ajan oranı kaydedilmiştir. Ayrıca bipolar montajda EEG sürekli olarak kaydedilmiş ve dijital olarak yüklenmiştir. Ham EEG 500 Hz ile sayısallaştınlmıştır. 5000 veri noktalı (10 sn) EEG bölümleri çalışmada kullanılan bireysel ölçümlerin hesaplanmasında kullanılmıştır. Veri tabanı toplam olarak 25 hastadan alman kayıtlarla oluşturulmuştur. Bulanık mantık sistemin üyelik fonksiyonu ve kural tabanı uzmanların gözetiminde belirlenmiştir. Operasyondan önce hastaların kan basıncı ve EEG ölçümleri kaydedilmiştir. Bu sistem, düşük kan gazı çözünürlüğüne sahip halojenize edilmiş uçucu anestezik olan sevofluran anestezik ajanı için dizayn edilmiştir. Veriler Sakarya ve Kütahya Devlet Hastanesinde sevofluran uygulanmış olan ASA-I VE ASA-II grubu hastalardan toplanmıştır. Geliştirilen bulanık sistemin girişleri EEG'nin güç spektral yoğunluğundan ve kan basıncından elde edilen ondört sayısal parametredir. Geliştirilen bulanık sistem dört bulanık çıkartım ünitesi içermektedir. Herbir ünite üç giriş değişkenine sahiptir. Geliştirilen bulanık sistemin giriş değişkenleri EEG 1-25 Hz bölgesi güç yüzdesi, EEG 26-50 Hz güç yüzdesi, EEG alfa oram, EEG beta oranı ve kan basıncı değişkenleri kullanılarak hesaplanmıştır. EEG ham datasmın güç spektral yoğunluğu (PSD) ve diğer parametreler MATLAB 6.5 programı kullanılarak elde edilmiştir. Dört bulanık çıkartım ünitesi çıkartım işlemini toplam 226 kuralı kullanarak gerçekleştirmekte ve dokuz giriş değişkenini değerlendirmektedir. Sistemin cevapları uzmanlar tarafından uygulanan gerçek değerlerle karşılaştırılmış ve onlar tarafından tatmin edici bulunmuştur.
CONTROL OF THE DEPTH OF ANESTHESIA USING FUZZY LOGIC IN GENERAL ANESTHESIA SUMMARY Key words: Depth of anesthesia, Fuzzy Logic Control, EEG monitorization. The aim of this study was to develop a fuzzy logic control system in order to control depth of anesthesia during operation using blood pressure and EEG recordings taken from the patient as system variables. During the operation, every five minutes blood pressure and rate of anesthetic agent were recorded. Also an eight channel EEG in a bipolar montage was recorded continuously and stored digitally. The raw EEG was digitized at 500 Hz. EEG epochs of 5000 data points (10 sec) were used for computation of individual measures employed in the study. The data base was constructed of the records from a total of 25 patients. The membership function and the rule base of the fuzzy logic system were determined under the inspection of specialists. Before the operation, blood pressure and EEG measures of the patients were all recorded. This system was designed for anesthetic agent, sevofiurane, which is a halogenated volatile anesthetic with a low blood gas solubility. In Sakarya and Kütahya State Hospital, data were collected from the ASAI and ASAII patients, who were administered sevofiurane. The inputs of the developed fuzzy system were fourteen numerical parameters computed from power spectral density of the EEG and blood pressure. The developed fuzzy system includes four fuzzy inference units. Every unit has three input variables. The input variables of the developed fuzzy system were obtained by using EEG 1-25 Hz range power percent, EEG 26-50 Hz range power percent, EEG beta ratio, EEG alpha ratio and blood pressure variables. The power spectral density (PSD) of the EEG raw data and other parameters were derived by using MATLAB 6.5 program. The output was advice for anesthetic rate. Four fuzzy inference unit implements inference operation using totally 226 rules and evaluates nine input variables. The responses of the system were compared to the real values applied by some specialists and they were found to be satisfactory. xviii
