Çalışmanın amacı; Sürdürülebilir Kalkınma Hedefleri kapsamında Dünya Sağlık Örgütü'nün 2007 Raporu'nda yer alan yapı taşlarını esas alarak sağlık sistemlerinin performansını ortaya koyarak sürdürülebilirliğini belirlemektir. Çalışmada Birleşmiş Milletlerin SKH göstergeleri, Sağlık Bakanlığının 2024- 2028 Stratejik Planı ve 12. Kalkınma Planı kapsamında belirlenen bir gösterge seti kullanılmıştır. Belirlenen gösterge seti WHO 2007 raporunda belirlenen sağlık sistemi yapı taşları çerçevesine uygun olarak sağlık harcamaları/finansmanı, sağlık insan gücü, tıbbi ürünler, aşılar ve teknoloji, hizmet sunumu, erişim, sağlığın geliştirilmesi, finansal risk koruması ve verimlilik başlıkları altında gruplandırılmıştır. Çalışmada 1990- 2022 verileri esas alınmış olup veriler WB ve OECD veri tabanlarından elde edilmiştir. Verilerin analizinde zaman serileri analiz yöntemlerinden üstel düzleştirme yöntemi kullanılmıştır. Çalışmanın bulgularına göre 2035 yılına kadar toplam sağlık harcamalarında İspanya dışındaki ülkelerde artış beklenirken Türkiye'de kamu sağlık harcamalarındaki artışların daha ciddi boyutlarda olacağı öngörülmektedir. İnsan gücü göstergelerinden kişi başına hekim ve hemşire sayılarında tüm ülkelerde artış beklenmesine karşın, diş hekimi, eczacı ve ebe sayılarında Türkiye dışındaki ülkelerde önemli bir değişim öngörülmemekte olup bu durumda Türkiye'nin mevcut sağlık insan gücünün yetersizliği rol oynamaktadır. Çalışmada kişi başına BT sayısında tüm ülkelerde ve MRI sayısında Birleşik Krallık dışındaki ülkelerde artış beklenmektedir. İlaç satışında Türkiye ve İspanya'da yatay seyir gözlenmesine karşılık diğer ülkelerde artış beklenmektedir. Hizmet sunumu göstergelerinden hastane sayısı ile ortalama kalış süresinde Birleşik Krallık'da ve yatak sayısında Türkiye'de artış beklenirken diğer ülkelerde genel eğilimin yatay ya da düşüş yönlü gerçekleşmesi beklenmektedir. Hekime başvuru sayısında Türkiye'de ciddi artış öngörülmesine karşılık diğer ülkelerde yatay seyir gözlenmektedir. Bağışıklama oranlarında bütün ülkelerin iyi bir performans göstereceği tahmin edilmektedir. Çalışmada Türkiye'de düşüş gözlenmekle birlikte anne ölüm oranları diğer ülkelerden daha yüksek oranda seyredecektir. Doğumda beklenen yaşam süresinin Türkiye'de artması beklenmektedir. Bebek ölüm oranlarında Türkiye'de daha yüksek olmak üzere tüm ülkelerde yatay bir seyir öngörülmektedir. Genel doğurganlık hızında diğer ülkelerde değişim beklenmezken Türkiye'de düşüş meydana geleceği tahmin edilmektedir. Finansal risk koruması göstergelerinden toplam cepten sağlık harcamasının Almanya, Birleşik Krallık ve İspanya'da; kişi başı cepten sağlık harcamasının ABD, Almanya ve İspanya'da artış göstermesi beklenirken cepten sağlık harcamalarının toplam sağlık harcamaları ve GSYİH'ya oranında önemli bir değişim beklenmemektedir. Toplam sağlık harcamalarının GSYİH'ya oranında Türkiye'de daha çok yatay seyir beklenirken, diğer ülkelerde artış öngörülmektedir. Kamu sağlık harcamalarının GSYİH'ya oranında Birleşik Krallık ve Fransa'da artış olması beklenirken Türkiye'de kısmi düşüş öngörülmektedir. Özel sağlık harcamalarının GSYİH'ya oranında ve kamu sağlık harcamalarının toplam sağlık harcamalarına oranında herhangi bir değişim beklenmezken kamu harcamalarından sağlık harcamalarına ayrılan payda Türkiye, Almanya, Fransa ve Birleşik Krallık'da artış öngörülmektedir. Çalışma sonucunda Türkiye'nin sağlık harcamaları, hekime başvuru sayısı, doğurganlık oranları gibi göstergelerinde diğer ülkelere göre daha dezavantajlı olduğu, önlem alınmaması halinde bu dezavantajın ileriki yıllarda da devam edeceği söylenebilir. Ancak cepten sağlık harcamaları ve sağlık harcamalarının GSYİH'daki oranında daha avantajlı görülmektedir. Sağlık politikacıları ve uygulayıcılarına dezavantajlı durumların ortadan kaldırılması ve avantajlı durumların daha da geliştirilmesi için politika belirleme ve planlama yapmaları önerilir.
The aim of the study is to determine the sustainability of health systems by revealing their performance based on the building blocks in the 2007 Report of the World Health Organisation within the scope of Sustainable Development Goals. In the study, a set of indicators determined within the scope of the United Nations SDG indicators, the 2024-2028 Strategic Plan of the Ministry of Health and the 12th Development Plan were used. The indicator set is grouped under the headings of health expenditures/financing, health manpower, medical products, vaccines and technology, service delivery, access, health promotion, financial risk protection and efficiency in accordance with the framework of health system building blocks determined in the WHO 2007 report. The study is based on 1990-2022 data and data are obtained from the WB and OECD databases. In the analysis of the data, exponential smoothing method, one of the time series analysis methods, was used. According to the findings of the study, while an increase in total health expenditures is expected in countries other than Spain until 2035, the increase in public health expenditures in Turkey is expected to be more significant. Although an increase is expected in the number of physicians and nurses per capita in all countries, no significant change is expected in the number of dentists, pharmacists and midwives in countries other than Turkey. In the study, an increase is expected in the number of CTs per capita in all countries and in the number of MRIs in countries other than the United Kingdom. Pharmaceutical sales are expected to remain flat in Turkey and Spain, whereas an increase is expected in other countries. Among service delivery indicators, the number of hospitals and average length of stay are expected to increase in the United Kingdom, and the number of beds is expected to increase in Turkey, while the general trend is expected to be flat or declining in other countries. While a significant increase is expected in Turkey in the number of visits to physicians, a flat trend is observed in other countries. All countries are expected to perform well in immunisation rates. In the study, although a decline is observed in Turkey, maternal mortality rates will be higher than in other countries. Life expectancy at birth is expected to increase in Turkey. Infant mortality rates are expected to remain flat in all countries, with a higher rate in Turkey. While the overall fertility rate is expected to remain unchanged in other countries, it is expected to decline in Turkey. Among financial risk protection indicators, total out of-pocket health expenditures are expected to increase in Germany, the United Kingdom and Spain, and per capita out-of-pocket health expenditures are expected to increase in the United States, Germany and Spain, while the ratio of out-of-pocket health expenditures to total health expenditures and GDP is not expected to change significantly. While the ratio of total health expenditures to GDP is expected to remain mostly flat in Turkey, it is expected to increase in other countries. The ratio of public health expenditures to GDP is expected to increase in the United Kingdom and France, while a partial decrease is expected in Turkey. While no change is expected in the ratio of private health expenditures to GDP and the ratio of public health expenditures to total health expenditures, the share of public expenditures allocated to health expenditures is expected to increase in Turkey, Germany, France and the United Kingdom. As a result of the study, it can be said that Turkey is more disadvantaged compared to other countries in indicators such as health expenditures, number of physician visits and fertility rates, and this disadvantage will continue in the coming years if no measures are taken. However, out-of-pocket health expenditures and the ratio of health expenditures to GDP are more advantageous. It is recommended that health policy makers and practitioners should make policy determination and planning for the elimination of disadvantageous situations and further development of advantageous situations.
